RESIGN MEMBERSHIP

  • THIS FORM MUST BE SUBMITTED TO WOODSIDE TO STOP DUES BILLING.

    I, the undersigned, understand that by completing this form my membership at Woodside will be resigned per the terms of the club.
    • Per your membership contract, to effectively resign your membership, Woodside requires 30 days notice prior to the 15th of the month for all membership change requests. Your account must be in good standing to resign your membership.
    • House charges will be prohibited upon submission of my resignation form.
    • My entire Woodside account balance must be paid at the time of my resignation submission, this balance may include: house charges, membership dues and early termination fees (if applicable).
    • Membership contracts will go to a month-to-month basis after the contract agreement unless a member submits for resignation after the contract is fulfilled.
    • By submitting a membership resignation you are agreeing to forfeit your enrollment fee. If you would like to temporarily pause your membership and not forfeit your enrollment fee please submit a suspension request here.
