  • This form must be submitted to Woodside to stop dues billing.

    I, the undersigned, understand that by completing this form my membership at Woodside will be terminated per the terms of the club.
    • Resignation forms must be submitted thirty (30) days prior to the 15th of the month.
    • House charges will be prohibited upon submission of my resignation form.
    • My entire Woodside account balance must be paid at the time of my resignation submission, this balance may include: house charges, membership dues and early termination fees (if applicable).
    • Membership contracts will go to a month-to-month basis after the contract agreement unless a member submits for resignation after the contract is fulfilled.

  • We strive to continually improve our services. Please take a moment to let us know the reason(s) for terminating your membership. Any feedback is greatly appreciated.
  • I understand that should I wish to rejoin Woodside, I am subject to the approval process that is in place at that time. Membership fees and dues will apply.
